Dr. Patel and staff are dedicated to providing you with a pleasant visit. Dr. Patel stays current on the latest and best Family Practice treatment options for his patients by participating in Continuing Education courses throughout the year. Our goal at Millgrove Medical Center is to work with our patients and their families to improve their health and overall well-being. We aim to establish and maintain a trusting physician-patient relationship in a caring environment. We want you to know that in our office you can expect to be treated with respect at all times. We strive to deliver the best diagnosis and treatment options. We believe that these goals can be met through a continuous and open dialogue between you and our team. We encourage you to involve yourself in maintaining your healthcare and to ask questions throughout our relationship. In our opinion, an informed and involved patient is most likely to achieve the highest level of healthcare. We look forward to working with you to achieve this goal. Our administrative staff is ready to help you with questions about scheduling, financial policy, and insurance, to make that part of the process as simple as possible.

1st VISIT:

On your first visit please bring the following:

  • Old medical records including immunization records
  • List of all medications with dosages
  • Health insurance card
  • The registration process will be facilitated if you download and bring in the completed health forms, which are found under online forms.

Important: A parent or guardian must accompany all patients under the age of eighteen.

We will schedule your appointment as promptly as possible. If you are sick or have an acute illness, every attempt will be made to see you on that day. We try our best to stay on schedule and minimize waiting; however, emergency cases can arise and cause some delays. We appreciate your understanding and patience.

FINANCIAL POLICY

PATIENT BILLING:

For you convenience, we accept all major credit cards including Visa, MasterCard, Discover, checks, and cash. Our goal is to deliver the best family practice healthcare at a reasonable cost to our patients. Therefore, payment is due at the time service is rendered unless other arrangements have been made in advance.

Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.

Most insurance companies respond within four to six weeks. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

INSURANCES:

At Millgrove Medical Center, we make every effort to provide you with the best healthcare options. You can expect your insurance company to reimburse for either a portion or all of our fee depending on the services provided. That portion is determined by the contract between you and your insurance company. At all times, you can be confident that we will always provide you with our best services without regard to the limitations imposed by your insurance coverage. To do otherwise would violate our contract with you – a contract we feel morally obligated to honor.

Please note that any applicable copayment is due at the time of the visit. If there is a deductible which has not been met or the claim is denied due to a limitation in coverage, the patient will be responsible for the balance.
