Yellow Fever

Yellow Fever vaccination can only be administered by a CDC Designated Yellow Fever provider. Dr Nilesh Patel is certified to administer the vaccination and provide the International Certificate of Vaccination. Immigration Physicals

Dr Nilesh V Patel is a USCIS Designated Civil Surgeon . Individuals applying for lawful permanent resident status (Citizenship, or a Green Card) are required to obtain an immigration physical prior to approval of their application.

The following vaccines are generally required  in adults over 18 years of age:

  • DT, DTP, DTaP, Td or Tdap
  • MMR
  • Varicella (Chicken Pox)
  • Influenza (During season)
  • Pneumococcal (for 65 years and older)

Form I-693

Generally, all applicants filing for adjustment of status to that of a lawful permanent resident must submit Form I-693 completed by a designated civil surgeon. Form I-693 is used to report results of a medical examination to USCIS. The examination is required to establish that an applicant is not inadmissible to the United States on public health grounds.
